Family Medicine Associates of Lincoln County will be providing sports physicals at West Lincoln Middle School on Wednesday, May 7. We will begin physicals around 1:00 and will be finished before school is dismissed. The cost is $20 and checks should be made payable to WLMS.
Your son/daughter received the medical history portion of the physical form. Please complete both sides of the form, sign the back page (both athlete and parent sign), and return the form with your payment by Friday, May 2.

REMINDER: Students must have a physical evaluation prior to participating in ANY tryouts or workouts for any sport. This is an opportunity to get a sports physical that will last the entire 2025-2026 school year. Physicals are only good for 395 days from the date received.
